Questions from the people who hand it out.
The ones that come up in the first meeting, answered plainly. If yours is missing, write to [email protected].
Getting started.
What does it cost us?
Nothing. Care Journal is free to the person using it and free to the organisation handing it out. We are funded through research partnerships, and those run on records people have separately chosen to share.
Does this touch our EHR?
No. It runs on the patient’s phone and the record belongs to them. There is nothing to install on your side and nothing for IT to approve.
Do we become a data controller?
No. The person holds their own record. If they choose to send you a summary, you hold that summary the way you would hold anything else a patient brings you.
How long does it take to roll out?
A QR code on a folder takes an afternoon. A link in the portal takes as long as your portal change process takes.
Safety and scope.
Does it grade symptoms?
Not for the patient. Care Journal records what someone says in their own words and keeps it dated. Severity is whatever they call it, and no clinical grade is shown to them.
What if someone reports something urgent?
Care Journal tells them to contact their care team or emergency services and says so in capitals. It never routes an alert to you, because nobody here is monitoring it.
Who can see a patient’s journal?
Nobody, until the patient sends it. Sharing is per item and can be withdrawn.
Is it a medical device?
No. It does not diagnose, prevent, mitigate, or treat any health condition, injury, illness or disease.
Ask us the one that is not here.
Free for patients and families. Nothing is shared with anyone unless you send it.
