Where does it come from?
One source: the person the record belongs to. People assemble their own record in Care Journal for their own reasons, then choose whether to share it for research.
What is actually in a record?
Diagnoses, labs, medications, vitals and payer detail pulled from the clinics that hold them, plus what the person writes and says between visits, dated as it happens.
How is it coded?
Structured on entry rather than mapped afterwards, so terms arrive analysis-ready.
How big is the cohort?
Growing every week. We would rather show you the shape of the data against your criteria than quote a number that is stale by the time you read it.
Consent and diligence.
Will the consent survive review?
It is consented at source, per study, with the sponsor named before anyone opts in. People can withdraw, and withdrawal propagates.
Is it de-identified?
Yes, before it reaches you, and re-identification is not available to buyers at any price.
Can we audit the chain?
Every record carries its provenance: which clinic it came from, when it arrived, and which consent covers it.

Is this a clinical trial dataset?
No. Trial capture is Healthnotes, a separate product on burna.ai. Care Journal data comes from people living ordinary lives who chose to share what they had.
